CONTRACT
TERMS AND AGREEMENTS
 
The client agrees to be responsible for making sure their pet is:

* up-to-date on all vaccines and has a Bordetella (kennel cough) shot.

* administered flea and tic treatments regularly.

* house trained and spayed/neutered. (Puppies “in training” are welcome, an   additional fee will be charged.)

* wearing a collar that has town license and rabies tags on it.

* Your dog also needs to bring his leash.

The client will provide the daycare facility with:

* a copy of the dog’s current vaccination and health record.

* a copy of the rabies certificate (this is required by state law).

* written instructions for medications, feeding and any other special considerations your dog requires. All meds and food must be safely sealed and clearly labeled.

* alternate/emergency contact information.

 

Ill dogs are asked to stay home until they are feeling better. This policy is intended for the health and benefit of all. Clients are to call their pet in sick the night before or the morning of the dog’s absence. A charge will not be made for your dog’s sick day if the notification call is made.

The establishment will not be held liable for any illness or injury your dog may acquire. If your dog needs medical care during his visit, you will be contacted immediately. The client will provide a Consent to Sign Form, allowing Elizabeth or Michael Baton to sign for immediate care for your pet if necessary.

All reasonable precautions will be made to make sure that your pet is safe and happy. American k-9 Daycare reserves the right to terminate the contract if your pet’s personality conflicts with the people or the other dogs in our home. The required pre-screening visitation should eliminate the occurrence of such an incident. If, however, a conflict occurs which involves your pet, you will be called immediately and you will be held responsible for removing your dog from the facility promptly.
